To register as a new patient please call into reception and ask for a New Registration Pack.

When you visit the surgery for the first time you will be required to bring in proof of your residency and identity and you will need your NHS Number to complete the registration forms.  We need to see a utility bill and photographic ID and for children, we need to see their birth certificate. 

You will also be asked to sign the form to confirm that the details are correct.

When you register you will also be asked to complete a medical questionnaire as it can take a considerable time for us to receive your medical records from your previous practice.

If you are pregnant at the time of registration please inform reception.
